MEDICAL AND DENTAL HISTORY FORMS
Orthodontic treatment, much like any medical procedure, requires a detailed medical and dental history to be completed by the patient or parent and reviewed by the orthodontist prior to treatment.
The answers to the questions will be helpful in selecting the safest and most effective means of providing your orthodontic care.
The forms will be available in the office at your initial examination, but in order to save time, you may print out the appropriate forms prior to your appointment and bring them in completed to expedite the process.
